I call this “the silent lesion.” 3. Symptomatic Apical Periodontitis: The tooth is tender to percussion. It may or may not have a periapical radiolucency.

Recognize the imaging features of complications due to periodontitis, includ-ing orbital, sinonasal, and intracranial sequelae. Discuss the char-acteristic imaging features of other odontogenic and non-odontogenic lesions that may cause lucent le-sions around the Epidemiology. Periapical cysts are typically seen in middle to older age (3 rd to 6 th decades) 2.. Pathology. Periapical cysts result from infection of the tooth, which spreads to the apex and into the adjacent bone. This leads to apical periodontitis, granuloma formation and eventual cyst formation.
Apical periodontitis refers to a spectrum of diseases that occur around the tooth apex, these can include 1: periapical granuloma; periapical abscess; periapical (radicular) cyst; Clinical presentation Complications. Apical periodontal disease of the maxillary teeth can extend into the maxillary sinuses with resultant odontogenic sinusitis. Periapical periodontitis or apical periodontitis is an acute or chronic inflammatory lesion around the apex of a tooth root, most commonly caused by bacterial invasion of the pulp of the tooth.

Conventional CT has considerably higher contrast resolution than that obtained with the modalities already mentioned and is ideal for evaluating complications due to periapical disease, although its spatial resolution is slightly less than that of cone-beam CT (11). Pathology. Periapical cysts result from infection of the tooth, which spreads to the apex and into the adjacent bone. This leads to apical periodontitis, granuloma formation and eventual cyst formation. These cysts are therefore centered on the apex of the tooth and tend to be small, most <1 cm. For this diagnosis the standard conventional intra‐oral radiographic exposures for diagnosing periapical periodontitis, usually periapical radiographs, are required, and if periodontal pathology is evident clinically a long‐cone paralleling technique 17 is preferred for the taking of the periapical radiographs. Apical periodontitis included radiographically diagnosed (periapical radiographs, panoramic radiographs or computerized tomography (CT)). Studies assessing the presence of apical periodontitis based on patients’ self‐report or a clinical examination without a radiographic assessment were excluded. Comparator (control)

Periapical cysts result from infection of the tooth, which spreads to the apex and into the adjacent bone. This leads to apical periodontitis, granuloma formation and eventual cyst formation. These cysts are therefore centered on the apex of the tooth and tend to be small, most <1 cm. There is also unsurprisingly usually overt evidence of caries. Apical periodontitis (AP) is a consequence of pulp necrosis caused by micro-organisms within the root canal system.

It is a likely outcome of untreated dental caries, and in such cases it can be considered a sequela in the natural history of tooth decay, irreversible pulpitis and pulpal necrosis. The lumen of the former is in continuity with the root canal lumen; the “true” cyst is dissociated from the root and may therefore be resistant to conventional root canal treatment (RCT) and need surgical extirpation. Commonly known as a dental cyst, the periapical cyst is the most common odontogenic cyst.It may develop rapidly from a periapical granuloma, as a consequence of untreated chronic periapical periodontitis..

‘Apical periodontitis’ is a general term used to describe the periapical inflammatory process that occurs in response to the presence of micro-organisms and other irritants within the root canal system of a tooth. Aim: To investigate the possible therapeutic effects of alpha-lipoic acid (ALA) in a model of chronic apical periodontitis in rats by analysing biochemical, histopathological and micro-CT parameters. Methodology: The study was approved by the Animal Ethics Committee of the Near East University. PRF preparation technique is very simple, inexpensive and doesn't require any chemical agents.
